This should be agreed between discharging and receiving teams.<\/li>\n<\/ul>\n<p><strong><span style=\"font-size: 18pt;\"><span style=\"color: #993300;\">Transfer to Referring Renal Unit<\/span><\/span><\/strong><\/p>\n<ul style=\"list-style-type: disc;\">\n<li>Contact the centre before and on day of discharge.<\/li>\n<li>A copy of the discharge letter (medical and nursing) with a computer printout of the biochemistry and haematology results should accompany patient on transfer and\/or faxed to receiving unit. NO <strong><span style=\"color: #993300;\">INFORMATION REGARDING THE DECEASED DONOR SHOULD BE INCLUDED IN THIS SUMMARY ACCOMPANYING THE PATIENT.<\/span><\/strong><\/li>\n<li>A patient transfer summary should also be completed and emailed to the parent unit.
