{"id":1546,"date":"2018-09-12T11:36:02","date_gmt":"2018-09-12T11:36:02","guid":{"rendered":"http:\/\/edren.org\/ren\/?page_id=1546"},"modified":"2025-03-25T16:36:10","modified_gmt":"2025-03-25T16:36:10","slug":"renal-transplant-biopsy","status":"publish","type":"page","link":"https:\/\/edren.org\/ren\/handbook\/transplant-handbook\/renal-transplant-biopsy\/","title":{"rendered":"Renal transplant biopsy"},"content":{"rendered":"<p><span style=\"font-family: georgia, palatino, serif;\">Renal allograft biopsies are performed under USS guidance in x-ray department by the radiologist.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">Urgent biopsy requests should be discussed with a Consultant Radiologist.\u00a0 If it is a non-urgent biopsy the Trak request needs to be completed and does not require further discussion.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">All patients must have:<\/span><\/p>\n<h3><span style=\"font-family: georgia, palatino, serif;\"><strong><span style=\"color: #993300;\">Consent<\/span><\/strong><\/span><\/h3>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">Must be obtained by the doctor requesting the biopsy<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Patient informed of risks<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Significant bleeding (requiring blood transfusion \/ further surgical intervention) is approximately 1-2%<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">The risk of graft loss is &lt;1 in 250 biopsies, and probably less in current era<\/span><\/li>\n<\/ul>\n<h3><span style=\"font-family: georgia, palatino, serif;\"><strong><span style=\"color: #993300;\">Blood tests<\/span><\/strong><\/span><\/h3>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">For biopsy to proceed results required:<\/span>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">platelets &gt; or = to 80 x 10<sup>9L<\/sup><\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">PT &#8211; prolongation of &lt; 3 seconds<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">\u00a0if patient on Warfarin an INR of &lt; or = 1.5. APTT normal<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Group and save<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">FBC<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><span style=\"font-family: georgia, palatino, serif;\">Heparin should be stopped the evening prior to the planned biopsy<\/span><br \/>\n<span style=\"font-family: georgia, palatino, serif;\">Aspirin \/ Warfarin discontinued<\/span><br \/>\n<span style=\"font-family: georgia, palatino, serif;\">Oral Fluids only<\/span><br \/>\n<span style=\"font-family: georgia, palatino, serif;\">Antibody samples should be sent to Tissue Typing on all patients receiving a biopsy<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">Results, Consent Form and Pathology Form <strong><span style=\"color: #993300;\">must <\/span><\/strong>be attached to front of case notes for the attention of radiologist (if samples are required for EM and immunofluorescence then this must be clearly indicated on the request card. These are required if de novo \/ recurrence of a primary glomerulonephritis is suspected).<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">Pathology Department contacted and told of the biopsy and arrangements made to collect the specimens.<\/span><\/p>\n<p><strong><span style=\"color: #993300; font-family: georgia, palatino, serif;\">Pathology request forms:<\/span><\/strong><\/p>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\"><strong><span style=\"color: #993300;\">Must <\/span><\/strong>be filled in by the doctor requesting the biopsy<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Clinical problem stated on the form<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Unless otherwise stated it will be assumed that samples for light microscopy and frozen section are required<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">\u00a0If sample for immunofluorescense or electron microscopy is required this <strong><span style=\"color: #993300;\">must<\/span><\/strong> be stated on the form<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Request form <strong><span style=\"color: #993300;\">must <\/span><\/strong>be attached to the front of the case notes<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Dr. Chris Bellamy is the contact person for pathology bleep 5044 or tel. 27150<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Weekend on call Pathologist can be contacted via RIE switchboard<\/span><\/li>\n<\/ul>\n<p><strong><span style=\"color: #993300; font-family: georgia, palatino, serif;\">Post biopsy observations should be recorded:<\/span><\/strong><\/p>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">every 15 mins for first 30 mins<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">\u00a0every 30 mins for 2 hours<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">4 hourly<\/span><\/li>\n<\/ul>\n<p data-cke-temp=\"1\" data-cke-filler-webkit=\"end\">\n","protected":false},"excerpt":{"rendered":"<p>Renal allograft biopsies are performed under USS guidance in x-ray department by the radiologist.
