For example due to metabolic adverse effects in a low\/standard rejection risk patient.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">At 3 months to remain on 5 mg ( some physicians will consider 7.5 mg if &gt;75 kg in weight). Generally patients remain on maintenance dose until the end of the first year and then review.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">At one year, cessation of Prednisolone may be considered (See steroid withdrawal protocol)<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\"><strong><span style=\"color: #993300;\">NB<\/span><\/strong> caution should be exercised in patients with an increased risk\u009d of rejection.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">All patients to receive Ranitidine (150 mgs od) along with Prednisolone.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">This schedule may be altered if rejection occurs.<\/span><\/p>\n<h5><strong><span style=\"font-family: georgia, palatino, serif; color: #993300;\">Increased Risk for Steroid withdrawal:<\/span><\/strong><\/h5>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">Level of HLA mismatching, particularly HLA-DR and DQ<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">&gt; 2 transplants<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">highly sensitised patients<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Rejection episodes &gt; 1 or more acute rejection episodes Banff grade &gt; II<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">De-novo proteinuria<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">Late acute rejection i.e. occurring after 6 months. In this case, steroid withdrawal is not recommended and, if indicated, should only be undertaken after renal biopsy has confirmed no ongoing inflammation or rejection.<\/span><\/li>\n<\/ul>\n<h5><strong><span style=\"font-family: georgia, palatino, serif; color: #993300;\">Steroid withdrawal<\/span><\/strong><\/h5>\n<p><span style=\"font-family: georgia, palatino, serif;\">Steroid withdrawal should be discussed with the patient and they should be informed of the risk of rejection.<\/span><\/p>\n<p><span style=\"font-family: georgia, palatino, serif;\">The steroids should be withdrawn according to the following schedule:<\/span><\/p>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">Decrease by 1 mg per month till 0 mg<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">The patient requires monthly blood for creatinine<\/span><\/li>\n<\/ul>\n<h5><strong><span style=\"font-family: georgia, palatino, serif; color: #993300;\">Steroid induced osteoporosis<\/span><\/strong><\/h5>\n<p><span style=\"font-family: georgia, palatino, serif;\"><strong><span style=\"color: #993300;\">All patients<\/span><\/strong> should receive additional elemental calcium, this may be as one or two tablets per day depending on dietary intake.<\/span><\/p>\n<ul style=\"list-style-type: disc;\">\n<li><span style=\"font-family: georgia, palatino, serif;\">If GFR\u00a0 &gt;30 mls\/min Calcichew D3 Forte should be used (2 tabs).<\/span><\/li>\n<li><span style=\"font-family: georgia, palatino, serif;\">If GFR &lt; 30 mls\/min Alfacalcidol and Calcichew should be used.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-family: georgia, palatino, serif;\">HRT should be used in individuals at risk of osteoporosis (unless patient has a contra-indication to HRT).<\/span><br \/>\n<span style=\"font-family: georgia, palatino, serif;\">All patients should be given advice on diet, weight, exercise, smoking cessation, skin surveillance.<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Prednisolone is normally reduced according to the following schedule: 20 mg x 1 month started on day 2 15mg x 1 month 10 mg x 1 month 5 mg x thereafter Sometimes we will reduce steroids quicker than this.
