Because of this, this are is covered in more detail than you will find in many places on the Internet.<a id=\"The_normal_GBM\"><\/a><\/p>\n<h5><strong><span style=\"color: #993300;\">The normal GBM<\/span><\/strong><\/h5>\n<p>The GBM forms the boundary between blood and urine. Across it, water and other small molecules from the blood are filtered. This is described a little more in our page on <a href=\"http:\/\/edren.org\/ren\/edren-info\/normal-kidneys\/\" data-cke-saved-href=\"[wblink154]\"><span style=\"font-family: Arial;\">normal kidneys<\/span><\/a>. The GBM is made of a meshwork of proteins and other constituents. <strong><span style=\"color: #993300;\">Type IV collagen<\/span><\/strong> and <strong><span style=\"color: #993300;\">laminin<\/span><\/strong> are present in the largest quantities. Some specialized subtypes of these molecules are only found in specialised basement membranes such as the GBM. It is caused by a defect in one of the specialised types of type IV collagen. The GBM deteriorates with time, leading to progressive loss of glomeruli, often with deafness. <a href=\"http:\/\/edren.org\/ren\/edren-info\/alport-syndrome\/alport-anti-gbm-disease\/\" data-cke-saved-href=\"[wblink125]\">Further information on Alport syndrome<\/a>.<a id=\"Thin_GBM_disease\"><\/a><\/p>\n<h5><strong><span style=\"color: #993300;\">Thin GBM disease<\/span><\/strong><\/h5>\n<p>Is exactly what it says. The GBM is thin, and must sometimes break, as it causes blood to appear in the urine. These measurements are not easy, and the normal thickness of the GBM varies a little with age. The approximate normal thickness is about one quarter of one thousandth of a millimetre.<\/p>\n<p>Some people with thin GBM disease carry an abnormal type IV collagen gene of the type that can cause <a href=\"[wblink125]\" data-cke-saved-href=\"[wblink125]\"><span style=\"font-family: Arial;\">Alport syndrome<\/span><\/a> (see above). However they also have a normal copy of the same gene, which is usually enough to prevent serious trouble (cells contain two copies of all genes, except for those genes carried on the X chromosome. Where a disease only occurs if both copies are abnormal it is called &#8216;recessive&#8217;. Cystic fibrosis is the most common recessive disorder). Only when two people who both have abnormal genes have children together is there a risk that their children could have Alport syndrome (a maximum of a 1 in 4 chance for each child). This happens rarely and accounts for a small minority of cases of Alport syndrome.<\/p>\n<p>Some people with thin GBM develop more severe kidney trouble. We haven&#8217;t found any good sources.<br \/>\n<span style=\"font-family: Arial;\"><a href=\"mailto:renal@ed.ac.uk?subject=thin%20GBM%20disease%20from%20EdREN\" data-cke-saved-href=\"mailto:renal@ed.ac.uk?subject=thin%20GBM%20disease%20from%20EdREN\">Let us know<\/a><\/span> if you do.<a id=\"Benign_familial_haematuria\"><\/a><\/p>\n<h5><strong><span style=\"color: #993300;\">Benign familial haematuria<\/span><\/strong><\/h5>\n<p>The name just means that there is an inherited cause for blood appearing in the urine, and that those who have it don&#8217;t usually come to any harm. <span style=\"font-family: Arial;\"><a href=\"#Thin_GBM_disease\" data-cke-saved-href=\"#Thin_GBM_disease\">Thin GBM disease<\/a><\/span> (see above) often appears to explain this condition, but there may be other possible explanations. If kidney biopsies have shown thin GBM disease in other family members, it will not usually be necessary to undertake it in everyone who has blood in the urine.<\/p>\n<p>There is a discussion of conditions causing blood to appear in the urine in our page on <a href=\"http:\/\/edren.org\/ren\/edren-info\/haematuria-hematuria\/\" data-cke-saved-href=\"[wblink141]\"><span style=\"font-family: Arial;\">haematuria<\/span><\/a>.<a id=\"Nail-patella_syndrome\"><\/a><\/p>\n<h5><strong><span style=\"color: #993300;\">Nail-patella syndrome<\/span><\/strong><\/h5>\n<p>This rare inherited condition has a surprising and unusual mix of different features. People who inherit it may have hardly anything to see, or they may have all of these. The most common features are:<\/p>\n<table class=\" cke_show_border\" border=\"0\" width=\"87%\" cellspacing=\"0\" cellpadding=\"2\">\n<tbody>\n<tr>\n<td bgcolor=\"#e6f7ed\"><strong><span style=\"color: #993300;\">Common abnormalities in nail patella syndrome:<\/span><\/strong><\/td>\n<\/tr>\n<tr>\n<td style=\"width: 501px; height: 36px; background-color: #e6f7ed;\" valign=\"top\" width=\"501\" height=\"36\"><span style=\"color: #993300;\"><strong><span style=\"color: #993300;\">Nails<\/span><\/strong>:<\/span> nails of the thumbs and of the first finger or two are abnormally small and badly formed. How many nails are affected, and how badly, varies<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"28\"><span style=\"color: #993300;\"><b><span style=\"color: #993300;\">Knee caps<\/span><\/b><\/span> (patellae) &#8211; are usually missing or underdeveloped<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"28\"><span style=\"color: #993300;\"><b><span style=\"color: #993300;\">Elbows <\/span><\/b><\/span>&#8211; often do not straighten properly (this is called a contracture)<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"28\"><b><span style=\"color: #993300;\">Iliac horns <\/span><\/b>&#8211; these can be seen on X-rays of the pelvis, but aren&#8217;t visible from the outside<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"50\"><span style=\"color: #993300;\"><b><span style=\"color: #993300;\">Other problems with joints and skeleton<\/span><\/b><\/span> are found in some people. These can include scoliosis (curvature of the spine), contracture of other joints besides the elbow, and congenital foot abnormalities. Aches and pains may sometimes be caused by these changes.<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"28\"><b><span style=\"color: #993300;\">Kidneys<\/span> <\/b>&#8211; can have an abnormality of the GBM, and occasionally fail (see below)<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" bgcolor=\"#e6f7ed\" width=\"501\" height=\"28\"><b><span style=\"color: #993300;\">Glaucoma <\/span><\/b>&#8211; has been found quite commonly in patients with nail patella syndrome<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>Inheritance of the disease is &#8216;autosomal dominant&#8217;, which means that on average, one in two of the children of someone with Nail Patella Syndrome will inherit the condition. However people vary very much in how severely they are affected &#8211; from badly, to hardly at all.<\/p>\n<p><strong><span style=\"color: #993300;\">Kidney disease<\/span><\/strong> does not always occur, and when it does it may be minor. Unfortunately in some people it is more serious and it may cause a protein leak (<a href=\"[wblink153]\" data-cke-saved-href=\"[wblink153]\"><span style=\"font-family: Arial;\">nephrotic syndrome<\/span><\/a>), and in time progress to kidney failure. No way to prevent this from happening is known. Treatment is the same as for nephrotic syndrome and kidney failure of other types.<\/p>\n<p><strong><span style=\"color: #993300;\">The gene<\/span><\/strong> causing Nail Patella Syndrome has been identified and is called LMX1B. This disease has been discovered relatively recently and unfortunately remains poorly understood. It often causes a protein leak and <a href=\"http:\/\/edren.org\/ren\/edren-info\/nephrotic-syndrome\/\" data-cke-saved-href=\"[wblink153]\"><span style=\"font-family: Arial;\">nephrotic syndrome<\/span><\/a>. Deterioration to kidney failure is common.<a id=\"Other_diseases_affecting_the_GBM\"><\/a><\/p>\n<h5><strong><span style=\"color: #993300;\">Other diseases affecting the GBM<\/span><\/strong><\/h5>\n<p>The GBM is thickened with normal basement membrane material in two common kidney disease, <span style=\"font-family: Arial;\"><a href=\"http:\/\/edren.org\/ren\/edren-info\/membranous-nephropathy\/\" data-cke-saved-href=\"[wblink151]\">membranous nephropathy<\/a> and <a href=\"http:\/\/edren.org\/ren\/edren-info\/diabetic-kidney-disease\/\" data-cke-saved-href=\"[wblink136]\">diabetic nephropathy<\/a><\/span> (kidney disease caused by diabetes).
