{"id":3465,"date":"2019-11-28T12:53:21","date_gmt":"2019-11-28T12:53:21","guid":{"rendered":"http:\/\/edren.org\/ren\/education\/textbook\/reflux\/"},"modified":"2020-08-12T11:37:57","modified_gmt":"2020-08-12T11:37:57","slug":"reflux","status":"publish","type":"page","link":"https:\/\/edren.org\/ren\/education\/textbook\/conditions-that-affect-the-kidney\/reflux\/","title":{"rendered":"Reflux"},"content":{"rendered":"<p><a href=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2019\/12\/reflux-pic.gif\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-3665 aligncenter\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2019\/12\/reflux-pic.gif\" alt=\"\" width=\"539\" height=\"298\" \/><\/a><\/p>\n<p>This is a chronic\u00a0<a href=\"http:\/\/edren.org\/ren\/education\/textbook\/interstitial-nephritis\/\">interstitial nephritis<\/a>\u00a0associated with vesico-ureteric reflux (VUR, usually congenital) in early life.\u00a0 There is also a close association with recurrent\u00a0<a href=\"http:\/\/edren.org\/ren\/education\/textbook\/uti\/\">urinary tract infections<\/a>\u00a0, and sometimes there are other urinary tract or other congenital abnormalities such as incomplete growth of the kidney &#8211;\u00a0<a href=\"http:\/\/edren.org\/ren\/education\/textbook\/inherited-congenital\/\">renal hypoplasia<\/a>.<\/p>\n<hr \/>\n<h3><strong><span style=\"color: #993300;\">Pathogenesis<\/span><\/strong><\/h3>\n<p>Normally the oblique entrance of the ureter through the bladder wall causes ureteric occlusion during bladder contraction. In VUR there is a structural abnormality of the intra-mural ureter allowing reflux of urine from the bladder to the kidney.\u00a0\u00a0<a href=\"http:\/\/edren.org\/ren\/education\/textbook\/interstitial-nephritis\/\">Chronic Interstitial Nephritis<\/a>\u00a0becomes established leading to further renal deterioration.\u00a0 By an uncertain mechanism, interstitial nephritis may cause focal scars and initiate a slowly progressive renal deterioration.<\/p>\n<hr \/>\n<h3><strong><span style=\"color: #993300;\">Clinical features<\/span><\/strong><\/h3>\n<p>Often asymptomatic. May present with hypertension, proteinuria reflecting renal damage; or with associated UTIs.\u00a0 Reflux itself sometimes causes loin pain during micturition but this is rarely a significant problem.<\/p>\n<hr \/>\n<h3><strong><span style=\"color: #993300;\">Investigation<\/span><\/strong><\/h3>\n<p>Diagnosis is confirmed by demonstration of focal renal scars (cross-sectional imaging or isotope renogram; ultrasound less reliable for this); will also reveal other abnormalities, e.g. obstruction. Urine culture necessary to show infection. Assess renal function, proteinuria etc. in usual way.<\/p>\n<hr \/>\n<h3><strong><span style=\"color: #993300;\">Management<\/span><\/strong><\/h3>\n<p>Associated UTIs should be managed in the usual way. Correct obstruction.\u00a0 There is no consistent benefit from procedures to correct reflux.
