We have a separate page providing <a href=\"http:\/\/edren.org\/ren\/edren-info\/coronavirus-covid-19\/\">information about COVID-19 for patients with kidney disease.<\/a><\/p>\n<p>&nbsp;<\/p>\n<h3><strong>High-risk groups<\/strong><\/h3>\n<p>In Scotland, &#8220;shielding&#8221; is no longer advised and so this terminology should be avoided.\u00a0 Preference now is to refer to &#8220;highest-risk group&#8221;.\u00a0 \u00a0It is important to keep an accurate list of this group (e.g. for management of local outbreaks \/ prioritisation of booster vaccination).\u00a0\u00a0Guidance about \/ for this highest risk group is published by the <a href=\"https:\/\/www.gov.scot\/publications\/covid-highest-risk\/pages\/overview\/\">Scottish Government<\/a>.<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>How to add a patient to the &#8220;highest-risk list&#8221;:<\/strong><\/span>\u00a0 E-mail covid.highrisk@nhslothian.scot.nhs.uk\u00a0and provide: forename, surname, address, postcode and high-risk group (and helpful to also give CHI).<\/p>\n<p>&nbsp;<\/p>\n<h3><strong>Vaccination<\/strong><\/h3>\n<p>Fantastic leaflet giving <a href=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2022\/08\/kidney-patients-resources-for-covid-briefing-paper-june-2022-english.pdf\">information for health professionals working with kidney patients<\/a> (Nov 2023).<\/p>\n<p>PHS are promoting the following key messages:<\/p>\n<ul style=\"list-style-type: circle;\">\n<li>For kidney patients, getting fully vaccinated against COVID-19 is the best way to protect against serious disease, being admitted to hospital or dying as a result of COVID-19 infection.<\/li>\n<li>It\u2019s important kidney patients have all the recommended doses of the COVID-19 vaccine, because they may get less protection from the vaccine than the wider population.<\/li>\n<li>Getting all the recommended doses of the COVID-19 vaccine helps increase the level of protection for kidney patients as they may not have generated a full immune response to the first two doses.<\/li>\n<\/ul>\n<p>The JCVI priority groups for Covid vaccination are given <a href=\"https:\/\/www.gov.uk\/government\/publications\/priority-groups-for-coronavirus-covid-19-vaccination-advice-from-the-jcvi-30-december-2020\/joint-committee-on-vaccination-and-immunisation-advice-on-priority-groups-for-covid-19-vaccination-30-december-2020\">here<\/a>.\u00a0 A more up-to-date list of priority vaccination groups in Scotland is given <a href=\"https:\/\/www.nhsinform.scot\/covid-19-vaccine\/invitations-and-appointments\/who-will-be-offered-the-coronavirus-vaccine\">here<\/a>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">How to help patients in relation to the COVID-19 vaccine:<\/span><\/strong><\/p>\n<ul style=\"list-style-type: circle;\">\n<li>Direct people to <u><a title=\"\" href=\"https:\/\/eur01.safelinks.protection.outlook.com\/?url=http%3A%2F%2Fwww.nhsinform.scot%2Fcovid19vaccine&amp;data=05%7C01%7CRobert.W.Hunter%40nhslothian.scot.nhs.uk%7C04e326b8e67e4cc51e0408da505abbec%7C10efe0bda0304bca809cb5e6745e499a%7C0%7C0%7C637910648025567261%7CUnknown%7CTWFpbGZsb3d8eyJWIjoiMC4wLjAwMDAiLCJQIjoiV2luMzIiLCJBTiI6Ik1haWwiLCJXVCI6Mn0%3D%7C3000%7C%7C%7C&amp;sdata=%2FV1RssbnNR%2B5hJgAwNYZ4NmoEvLBhBj56Q4ULeFoZW8%3D&amp;reserved=0\" target=\"_blank\" rel=\"noopener noreferrer\" data-auth=\"VerificationFailed\" data-linkindex=\"3\">www.nhsinform.scot\/covid19vaccine<\/a>\u00a0<\/u>for up-to-date information about COVID-19 vaccination.<\/li>\n<li>Reassure your patients that community vaccination clinics will be safe and welcoming. see <a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/infections-and-poisoning\/coronavirus-covid-19\/coronavirus-covid-19-treatments\">NHS Inform<\/a>.\u00a0 The phone number for Lothian is <a href=\"tel:+44-300-790-6769\">0300 790 6769<\/a>.<\/p>\n<p>Treatments are now well-established (e.g. see NICE guidelines &#8211; link below).\u00a0 <span style=\"font-family: 'Source Sans Pro';\">Early<\/span><strong>\u00a0anticipatory care planning<\/strong><span style=\"font-family: 'Source Sans Pro';\">\u00a0is vital for ALL patients. \u00a0Complete NHS Lothian\u00a0<a href=\"http:\/\/intranet.lothian.scot.nhs.uk\/Directory\/TreatmentEscalationPlans\/Pages\/default.aspx\">Treatment Escalation Plan<\/a>\u00a0(TEP) on Trak<\/span><span style=\"font-family: 'Source Sans Pro';\">.\u00a0 The\u00a0<\/span><a href=\"https:\/\/isaric4c.net\/risk\/\">ISARIC 4C<\/a><span style=\"font-family: 'Source Sans Pro';\">\u00a0score can be an adjunct in prognostication.\u00a0\u00a0<\/span><span style=\"font-family: 'Source Sans Pro';\">All patients should be given the opportunity to participate in\u00a0<\/span><strong>RCTs<\/strong><span style=\"font-family: 'Source Sans Pro';\">\u00a0(such as\u00a0<\/span><a href=\"https:\/\/www.recoverytrial.net\">RECOVERY<\/a><span style=\"font-family: 'Source Sans Pro';\">).<\/span><\/p>\n<p>Many patients with kidney disease are eligible for treatment with neutralising monoclonal antibodies (nMABs): both in hospital and in the community.\u00a0 Non-hospitalised patients will be prioritised if they meet the following criteria:<\/p>\n<ul style=\"list-style-type: disc;\">\n<li>recipient of kidney transplant (including failed transplants within 12 months) who received B-cell depleting therapy within 12 months or were not vaccinated prior to transplantation or have an additional substantial risk factor<\/li>\n<li>non-transplant patients who have received a comparable level of immuosuppression<\/li>\n<li>CKD4 or 5 (eGFR &lt; 30) even without prescribed immunosuppression<\/li>\n<\/ul>\n<p>In NHS Lothian, non-dialysis patients eligible for these treatments will be identified and offered treatment via Public Health Scotland or enrolled in the PANORAMIC trial.\u00a0 Dialysis patients eligible for IV Sotrovimab will receive this at the end of dialysis treatment, usually in the WGH facility (see unit protocol below).<\/p>\n<h5><\/h5>\n<h5><em>Treatment in kidney disease<\/em><\/h5>\n<p>No major modification required to standard treatment.\u00a0 RAS inhibitors (ACEi \/ ARBs) should be continued.<\/p>\n<p>&nbsp;<\/p>\n<h3 style=\"font-family: 'Source Sans Pro';\"><strong>Information for patients<\/strong><\/h3>\n<p>See our page on\u00a0<a href=\"http:\/\/edren.org\/ren\/edren-info\/coronavirus-covid-19\/\">patient information about COVID-19<\/a>\u00a0and the\u00a0<a href=\"https:\/\/www.kidneycareuk.org\/news-and-campaigns\/coronavirus-advice\/\">Kidney Care UK site<\/a>.\u00a0 Having CKD &#8211;
