Therefore control of hypertension is important to stop the progression.<\/p>\n<p>Roughly speaking there are 4 main drug classes used in the management of hypertension. These are<\/p>\n<ol>\n<li>ACE &#8211; inhibitors<\/li>\n<li>Calcium Channel Blockers<\/li>\n<li>Diuretics<\/li>\n<li>Beta-Blockers<\/li>\n<\/ol>\n\n\n<p>This page hopes to explain a little about the common classes of medications,  when they are required and some of their most common side effects.<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><span style=\"color:#e31414\" class=\"tadv-color\"><strong>ACE -Inhibitors<\/strong><\/span><\/p>\n\n\n\n<p>ACE- Inhibitors (or angiotensin converting enzyme inhibitors to give them their Sunday name) are a commonly utilised medication in the management of hypertension. <\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"574\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-1024x574.png\" alt=\"\" class=\"wp-image-4903\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-1024x574.png 1024w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-300x168.png 300w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-768x430.png 768w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-1536x860.png 1536w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS-150x84.png 150w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/RASS.png 1844w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p>Above is the renin angiotensin aldosterone system ACE inhibitors  work between the stage of Angiotensin 1 which is inactive being converted to the active form of angiotensin 2. This breaks the chain and thus ultimately lowers the blood pressure.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"686\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1-1024x686.png\" alt=\"\" class=\"wp-image-4907\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1-1024x686.png 1024w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1-300x201.png 300w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1-768x514.png 768w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1-150x100.png 150w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ACE-i-1.png 1413w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>ACE- inhibitors = lisinopril, ramipril, catopril<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#de1111\" class=\"tadv-color\">Who do you think could benefit from ACE-inhibitor therapy?<\/span><\/strong><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-381\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Diabetics<\/strong><\/button><div id=\"ac-381\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">These medications are commonly prescribed to diabetic patients. They help to reduced proteinuria and thus progression of renal disease. They also are used as part of secondary prevention for cardiovascular disease which diabetics and those with renal disease are at increased risk of developing.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-42632\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Heart Failure<\/strong><\/button><div id=\"ac-42632\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Again these medications are commonly used in heart failure due to there role in reducing vasoconstriction and decreasing systemic vascular resistance.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-88617\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Elderly Patients<\/strong><\/button><div id=\"ac-88617\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">While you may see some older individuals on ACE-inhibitors, they are not first line and if you are treating an individual with a new diagnosis, this would not be the first medication you would prescribe. This is due to decreased renin production as individuals age.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-8730\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>All people with chronic renal impairment<\/strong><\/button><div id=\"ac-8730\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Whilst ACE-Inhibitors are useful in many causes of chronic kidney disease it is important to remember that it is <span style=\"text-decoration: underline;\" class=\"ek-underline\">contraindicated<\/span> in patients with renal artery stenosis.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-62683\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Black People<\/strong><\/button><div id=\"ac-62683\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Black individuals, along with the elderly, tend to have decreased renin production so are not as useful in the management of hypertension in these individuals.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#e11515\" class=\"tadv-color\">What are the side effects of ACE-inhibition?<\/span><\/strong><\/p>\n\n\n\n<p>As with all medications, ACE-inhibitors come with side effects. Below are listed a range of side effects. Select those that apply.<\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-69502\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Acute Kidney Injury<\/strong><\/button><div id=\"ac-69502\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This is a possible complication of ACE-inhibition in people who have renal artery stenosis. This is due to the fact that glomerular filtration  in those with renal artery stenosis is maintained by angiotensin-2 mediated constriction of the efferent arteriole which ACE-inhibition prevents. For this reason all individuals should have renal function monitoring following initiation of ACE-inhibition 1-2 weeks after implementation.<\/p>\n\n\n\n<p><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-25756\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Cough<\/strong><\/button><div id=\"ac-25756\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This is a common side-effect of ACE-inhibitors. It is due decreased degradation of bradykinin in the body (Angiotensin converting enzyme is also needed to convert bradykinin to an inactive peptide).  For most individuals switching to an angiotensin 2 receptor blocker can overcome this side effect<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-19244\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Angioedema<\/strong><\/button><div id=\"ac-19244\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This is another side effect of ACE-inhibition which occurs in 0.1% &#8211; 0.35% of recipients (higher percentage in black individuals). This again occurs due to  decreased bradykinin degradation, increased substance P and increased active Bradykinin metabolites. This leads to vasodilation and increased vascular permeability and ultimately angioedema<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-31104\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Erectile Dysfunction<\/strong><\/button><div id=\"ac-31104\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This is not a side effect of ACE-inhibition but can be attributed to other classes of antihypertensives.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-72047\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Hyperkalaemia<\/strong><\/button><div id=\"ac-72047\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Yes. Remember that Aldosterone works on the sodium potassium pump drawing potassium out and into the urine. With ACE -inhibition there is less aldosterone and thus plasma potassium is elevated.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p>As a last note on ACE-inhibitors, due to their side effect profile, it is important to <strong>withhold <\/strong>these medications if a patient is in a dehydrated state or is unwell. You should also be advising patients that these should be withheld if vomiting or unwell in order to prevent complications from profound hyperkalaemia or acute kidney injury.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#d51818\" class=\"tadv-color\">Calcium Channel Blockers<\/span><\/strong><\/p>\n\n\n\n<p>Calcium channel blockers are the first choice medication in black and older individuals. They work by decreasing calcium influx into cells leading to decreased vascular contractility (vasodilation) and by this mechanism blood pressure is reduced<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"754\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1-1024x754.png\" alt=\"\" class=\"wp-image-4931\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1-1024x754.png 1024w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1-300x221.png 300w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1-768x565.png 768w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1-150x110.png 150w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/ccb-1.png 1257w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p>Blood pressure is therefore reduced as cardiac output remains the same and vascular resistance drops.<\/p>\n\n\n\n<p>Calcium Channel blockers = Felodipine, nifedipine, amlodipine, diltiazem, verapamil<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#de1010\" class=\"tadv-color\">Who may benefit from taking a calcium channel blocker?<\/span><\/strong><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-26196\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>People with heart disease<\/strong><\/button><div id=\"ac-26196\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">As calcium channel blockers lead to vasodilation and decreased vascular resistance and arterial pressure. They are there effective in reducing cardiovascular and cerebrovascular disease.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-98033\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Older adults<\/strong><\/button><div id=\"ac-98033\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Calcium channel blockers are used as a first choice in the treatment of hypertension in those over 55 years.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-82160\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>People with heart failure<\/strong><\/button><div id=\"ac-82160\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">As calcium channel blockers are negatively inotropic and chronotropic, they should be avoided in people with impaired left ventricular function.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-79148\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>People with Raynaud&#8217;s phenomenon<\/strong><\/button><div id=\"ac-79148\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Calcium channel blockers can be useful to treat individuals who have Raynaud&#8217;s due to their action in preventing vasoconstriction.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#de1313\" class=\"tadv-color\">Side Effects<\/span><\/strong><\/p>\n\n\n\n<p>Common side effects of calcium channel blockers include ankle swelling &#8211; particularly with amlodipine and this tends to resolve with discontinuation of the medication. There are some further side effects. Select those which are correct<\/p>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-47305\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Headache<\/strong><\/button><div id=\"ac-47305\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This is a common side effect due to vasodilation in the cerebral arteries.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-92492\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Renal Impairment<\/strong><\/button><div id=\"ac-92492\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Renal impairment is not commonly seen due to calcium channel blockade however note that it can occur due to hypotension.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-48150\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Bradycardia<\/strong><\/button><div id=\"ac-48150\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">This can occur due to reduced formation of  and conduction of impulses through the cardiac conduction system which can lead to bradycardia.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-20642\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Angioedema<\/strong><\/button><div id=\"ac-20642\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">As stated above Calcium channel blockers can lead to peripheral oedema but not to angioedema.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-7218\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Bronchospasm<\/strong><\/button><div id=\"ac-7218\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">Bronchospasm is not associated with calcium channel blockers but is associated with another form of antihypertensive.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong>Thiazide Diuretics<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"657\" height=\"515\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/thiazide-diuretics.png\" alt=\"\" class=\"wp-image-4933\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/thiazide-diuretics.png 657w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/thiazide-diuretics-300x235.png 300w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/07\/thiazide-diuretics-150x118.png 150w\" sizes=\"auto, (max-width: 657px) 100vw, 657px\" \/><\/figure>\n\n\n\n<p>Therefore blood pressure is reduced due to  decreased cardiac output from reduced sodium reabsorption.<\/p>\n\n\n\n<p>Thiazide diuretics = bendroflumethiazide, chlorthalidone, indapamide<\/p>\n\n\n\n<p>Thiazides are another agent that can be used in the management of hypertension in those older than 55.<\/p>\n\n\n\n<p>Select further which of these further possible uses of Thiazide diuretics are correct.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-16711\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Thiazide diuretics are only useful as second or third line agents<\/strong><\/button><div id=\"ac-16711\" class=\"c-accordion__content\">\n<p class=\"has-background has-very-light-gray-background-color\">As stated above this is not true. Along with calcium channel blockers these can be thought of as first line agents for those over the age of 55years.
