A clinical examination showed that she had a ruptured aortic aneurysm leading to shock and circulatory failure.<\/p>\n\n\n\n<figure class=\"wp-block-table\">\n<table style=\"width: 100%; height: 235px;\">\n<tbody>\n<tr style=\"height: 235px;\">\n<td style=\"background-color: #e6f7ed; height: 235px;\">\n<h5><span style=\"color: #993300; font-size: 18pt;\"><span style=\"color: #993300;\"><span style=\"color: #ff0000;\">Her results were:<\/span><br \/><\/span><\/span><\/h5>\n<h5><span style=\"font-size: 18pt;\">[H<sup>+<\/sup>] =\u00a090 nmol\/L\u00a0\u00a0 pH = 7.05<\/span><br \/><span style=\"font-size: 18pt;\">P<sub>aCO2<\/sub>\u00a0=\u00a07.5 kPa<\/span><br \/><span style=\"font-size: 18pt;\">[HCO<sub>3<\/sub>] =\u00a015 mmol\/L<\/span><\/h5>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/figure>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#e11212\" class=\"tadv-color\">What is his acid base disturbance in this case? <\/span><\/strong><\/p>\n\n\n\n<p><em>(Choose one of these and then click to see if you are correct)<\/em><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"true\" data-scroll-offset=\"0\"><button id=\"at-12888\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Compensated Metabolic Acidaemia<\/strong><\/button><div id=\"ac-12888\" class=\"c-accordion__content\">\n<p class=\"has-very-light-gray-background-color has-background\">Compensated metabolic acidaemia is expected but due to his comatose state hypoventilation takes place resulting in respiratory acidaemia.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-77409\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Mixed metabolic acidaemia<\/strong><\/button><div id=\"ac-77409\" class=\"c-accordion__content\">\n<p class=\"has-very-light-gray-background-color has-background\"> Circulatory failure result in metabolic acidaemia and hypoventilation results in respiratory acidaemia.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-89568\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Respiratory Alkalaemia<\/strong><\/button><div id=\"ac-89568\" class=\"c-accordion__content\">\n<p class=\"has-very-light-gray-background-color has-background\">Yes, he has got a metabolic acidaemia but you would expect a compensatory respiratory alkalaemia which is not present in this case because of his comatose state resulting in hypoventilation.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-88250\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Metabolic acidosis<\/strong><\/button><div id=\"ac-88250\" class=\"c-accordion__content\">\n<p class=\"has-very-light-gray-background-color has-background\">You are partially correct but this is a mixed acid base disturbance.<\/p>\n<\/div><\/div>\n\n\n\n<p>\u00a0<\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-15607\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>General Feedback<\/strong><\/button><div id=\"ac-15607\" class=\"c-accordion__content\">\n<p class=\"has-background\" style=\"background-color:#f1f1f1\">Circulatory failure and shock (following a ruptured aortic aneurysm, in this case) result in metabolic (= lactic) acidaemia. This is due to hypovolaemia and poor circulation leading to a net anaerobic state. She is hypoventilating as a consequence of his comatose state, causing the respiratory acidaemia. Blood pressure is linked to circulating volume and as this falls, so too does blood pressure. With decreased blood pressure, ischaemia of major organs is a real possibility. These can include kidney, liver and brain all of which are vulnerable to ischaemia and without intervention may lead to catastrophic damage. This may be managed by replacement of fluids and inotropic support.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-20439\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Intravenous fluids<\/strong><\/button><div id=\"ac-20439\" class=\"c-accordion__content\">\n<p class=\"has-background\" style=\"background-color:#f2f2f2\">Whilst, as above it is important to replace the fluid volume lost to the extravascular space there may be a better answer to this question.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-95545\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Blood Products<\/strong><\/button><div id=\"ac-95545\" class=\"c-accordion__content\">\n<p class=\"has-background\" style=\"background-color:#f9f9f9\">This is a life threatening bleed, as such, activation of the major haemorrhage protocol (Acessed by 2222) and use of various blood products would be required to resuscitate this lady. Make sure you are aware of who you will have at your disposal on activation of this protocol. <\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-82609\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Cardiothoracic\/ Vascular Surgery<\/strong><\/button><div id=\"ac-82609\" class=\"c-accordion__content\"><\/div><\/div>\n\n\n\n<p class=\"has-background\" style=\"background-color:#f0f0f0\">This will be the definitive treatment for this lady and the fastest she can be taken to theatre the better prognosis she will have. Treatment is usually via a graft to replace the ruptured anneurysm. Note in this case it would be done as  emergency surgery, there is a screening program for anyone with a known AAA and with these individuals it may be possible to plan an elective procedure prior to rupture.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Unfortunately despite the above measures, this lady was taken to theatre where she arrested and died. Due to her advanced age her physiological reserve was low.
