You have a look on Trak and see that Mr Zhang was admitted with general lethargy, mild fever and just generally unwell. He had been investigated by his GP who found that his kidney function was a little deranged but identified no infectious source. He has underlying chronic renal disease which is thought to be linked to un-controlled blood pressure in the past. The blood tests were a routine admission.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-72049\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Insulin and Dextrose<\/strong><\/button><div id=\"ac-72049\" class=\"c-accordion__content\">\n<p>Whilst potassium is a little high, you could probably monitor it at this level. It would be good to get an ECG to check there is no abnormality there and this would help you decide if you needed to treat it at this level.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-91332\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Look at the drug chart<\/strong><\/button><div id=\"ac-91332\" class=\"c-accordion__content\">\n<p>Remember Mr Zhang has just had an MI and is likely to have been started on quite a few new medications for secondary prevention. Have a think about what medications could cause renal impairment post-MI.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-86245\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Go and see the patient<\/strong><\/button><div id=\"ac-86245\" class=\"c-accordion__content\">\n<p>This is rarely a wrong answer and whilst unexpected blood tests can be a sign of something wrong, without seeing the patient it is difficult to have a meaningful interpretation of these. Remember treat the patient not just the numbers!<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-20393\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Ring the Renal Reg on Call<\/strong><\/button><div id=\"ac-20393\" class=\"c-accordion__content\">\n<p>Whilst this may be part of your management plan, I think there is a little more information that should be gathered prior to ringing them for advice.<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#e70a0a\" class=\"tadv-color\">So what do you find out &#8211; click reveal to find out information about Mr Zhang.<\/span><\/strong><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-72751\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Reveal<\/strong><\/button><div id=\"ac-72751\" class=\"c-accordion__content\">\n<p>PC \u2013 Generally unwell and abnormal renal function<\/p>\n\n\n\n<p>HPC &#8211; Feeling generally unwell following MI 4 weeks ago. Had been well following discharge and felt like was progressing slowly however for last 2 weeks has had general malaise, mild myalgia and feeling a little feverish. No infective symptoms. No chest pain, SoB. No cough or coronavirus contact.<\/p>\n\n\n\n<p>PMHx \u2013 Hypertension, hypercholesterolaemia, COPD, MI- 2x stents to LAD<\/p>\n\n\n\n<p>Medications<\/p>\n\n\n\n<p>Lisinopril 10mg od<\/p>\n\n\n\n<p>Amlodipine 10mg od<\/p>\n\n\n\n<p>Bisoprolol 2.5mg od<\/p>\n\n\n\n<p>Aspirin 75mg od<\/p>\n\n\n\n<p>Clopidogrel 75mg od<\/p>\n\n\n\n<p>Salbutamol inhaler prn<\/p>\n\n\n\n<p>Atorvastatin 40mg od<\/p>\n\n\n\n<p>Trellegy 1 inh\/day<\/p>\n\n\n\n<p>Allergies \u2013 penicillin<\/p>\n\n\n\n<p>PMHx \u2013 Father MI in 50\u2019s. Mother \u2013 vascular dementia<\/p>\n\n\n\n<p>Social \u2013 Smokes 20 a day for 40 years, drinks 5 pints 5 nights a week, retired, lives sedentary lifestyle.<\/p>\n\n\n\n<p>You go to see the patient<\/p>\n\n\n\n<p>Feeling a bit nauseous, no chest pain. Alert, eating and drinking<\/p>\n\n\n\n<ul class=\"wp-block-list\" type=\"A\"><li>A- Maintaining own<\/li><li>B &#8211; Chest hyperinflated, mild creps bilaterally AE, Sats 89% RA \u2013 Normal for patient, No increased WoB<\/li><li>C- Warm peripherally, HS 1+2+0 nil added sounds. BP 115\/63, HR 65, pulses palpable, no radio\/radio delay. No bruits,  no sacral oedema, no peripheral oedema, JVP not visible, mucus membranes moist<\/li><li>D- Temp 37.8, Abdo soft no organomegaly, passing urine<\/li><\/ul>\n\n\n\n<p>Bloods &#8211; Hb 135, WCC 10.7, eosinophils 0.55 x 10<sup>9<\/sup> \/l , Plt 258 CRP 15<\/p>\n\n\n\n<p>CXR mild bilateral pleural effusions<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-50051\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>What is your differential diagnosis currently with the information you have?<\/strong><\/button><div id=\"ac-50051\" class=\"c-accordion__content\">\n<p>Allergy &#8211;  there is some signs of inflammation and eosinophillia so this is a possibility<\/p>\n\n\n\n<p>Infections &#8211; there is mild pyrexia and high normal WCC, although there is no sign of sepsis it wouldn&#8217;t be impossible he had an infection<\/p>\n\n\n\n<p>AKI &#8211; Secondary to ACE-Inhibitor introduction\/Uptitration. We would need to know if he was already being treated with ACE-inhibitor prior to his MI<\/p>\n\n\n\n<p>AKI &#8211; related to contrast nephropathy &#8211; usually occurs closer to time of contrast exposure but is a possibility<\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p>You are about to go and get some help because you are unsure as to what exactly is going on with this gentleman when the nurse asks you &#8211; Have you seen the rash on his foot.<\/p>\n\n\n\n<p>You go to have a look<\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-95604\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Click to Pull back the covers<\/strong><\/button><div id=\"ac-95604\" class=\"c-accordion__content\">\n<figure class=\"wp-block-image is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/upload.wikimedia.org\/wikipedia\/commons\/f\/fa\/Livedo_reticularis_of_left_leg.jpg\" alt=\"\" width=\"478\" height=\"232\"\/><figcaption>Picture from wikimedia commons<\/figcaption><\/figure>\n\n\n\n<p>Rash is present bilaterally, feet are cool peripherally. neurovascularly intact<\/p>\n<\/div><\/div>\n\n\n\n<p class=\"has-medium-font-size\"><strong><span style=\"color:#ee0a0a\" class=\"tadv-color\">What is this and does this change your differential diagnosis?<\/span><\/strong><\/p>\n\n\n\n<p>You call the med reg as your SHO is busy with another sick person. He also has myalgia and pyrexia. No coronavirus contacts.<\/p>\n<\/div><\/div>\n\n\n\n<p>B- Recent MI and angiography with 2x stents inserted, hypertension and hypercholesterolaemia. Strong family history of vascular disease.<\/p>\n\n\n\n<p>A &#8211; He is alert and well. Euvolaemic in terms of fluid balance. He has no cardiovascular or GI abnormalities. His respiratory exam is largely normal but he does have small bilateral pleural effusions. He has a mildly elevated temp at 37.8.
