This is based on creatine increase or urine output decrease.<\/p>\n\n\n\n<p>Stage 1 = Cr x 1.5-1.9 or increase of \u226526\u00b5mol\/l ; UO &lt;0.5ml\/kg\/h over 6 consecutive hours<\/p>\n\n\n\n<p>Stage 2 = Cr x 2.0 -2.9; UO &lt;0.5ml\/kg\/h over 12 hours<\/p>\n\n\n\n<p>Stage 3 = Cr x 3 or increase of \u2265354\u00b5mol\/l; UO &lt;0.3ml\/kg\/h over 24hours or anuria over 12 hours<\/p>\n\n\n\n<p>There are many other classifications which are similar (RIFLE, AKIN) which you can look at if you want further knowledge.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n<\/div><\/div>\n\n\n\n<p>You do not know the gentleman whose results are shown. <\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-40089\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Click to read his details from the computer system<\/strong><\/button><div id=\"ac-40089\" class=\"c-accordion__content\">\n<p>Mr Sparks 63 year old gentleman who was admitted yesterday.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"534\" height=\"676\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Medical-history-1.png\" alt=\"\" class=\"wp-image-5268\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Medical-history-1.png 534w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Medical-history-1-237x300.png 237w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Medical-history-1-118x150.png 118w\" sizes=\"auto, (max-width: 534px) 100vw, 534px\" \/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"530\" height=\"699\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Assesment.png\" alt=\"\" class=\"wp-image-5264\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Assesment.png 530w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Assesment-227x300.png 227w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/Assesment-114x150.png 114w\" sizes=\"auto, (max-width: 530px) 100vw, 530px\" \/><\/figure>\n<\/div><\/div>\n\n\n\n<p class=\"has-text-color has-medium-font-size\" style=\"color:#e20c0c\"><strong>Which on the regular medications should be withheld?<\/strong><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-76739\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Metformin<\/strong><\/button><div id=\"ac-76739\" class=\"c-accordion__content\">\n<p>Whilst this will not cause AKI in patients who take this during ill health, it can accumulate in AKI and lead to lactic acidosis so should be withheld in acute illness.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-53516\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Amlodipine<\/strong><\/button><div id=\"ac-53516\" class=\"c-accordion__content\">\n<p>This does not routinely need to be stopped in AKI however if the patient is still hypotensive it might be prudent to withhold this for a few days.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-99176\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Paracetamol<\/strong><\/button><div id=\"ac-99176\" class=\"c-accordion__content\">\n<p>This is not routinely stopped in people with AKI and is used as an effective antipyretic. Unlike NSAIDs which do need to be withheld in AKI.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-72173\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Ramipril<\/strong><\/button><div id=\"ac-72173\" class=\"c-accordion__content\">\n<p>As an ACE-inhibitor ramipril needs to be stopped in AKI. They reduce renal perfusion and as such with AKI can cause further damage. They can also lead to hyperkalaemia.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-91285\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Empagliflozin<\/strong><\/button><div id=\"ac-91285\" class=\"c-accordion__content\">\n<p>This is a sodium-glucose co-transporter-2 inhibitor (SGLT-2) causing patients treated with them to have glycosuria. There are two reasons why these should be stopped. The first is the risk of ketoacidosis in dehydrating illness but also as this gentleman has had a UTI &#8211; this is a contraindication for this. They can also lead to AKI.<\/p>\n<\/div><\/div>\n\n\n\n<p>You review the gentleman&#8217;s drug chart. Gentamicin has been calculated and prescribed on a separate chart and he is has been given amoxicillin 1g tds.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"469\" height=\"604\" src=\"http:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/medicine-chart.png\" alt=\"\" class=\"wp-image-5269\" srcset=\"https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/medicine-chart.png 469w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/medicine-chart-233x300.png 233w, https:\/\/edren.org\/ren\/wp-content\/uploads\/2020\/12\/medicine-chart-116x150.png 116w\" sizes=\"auto, (max-width: 469px) 100vw, 469px\" \/><\/figure>\n\n\n\n<p class=\"has-text-color has-medium-font-size\" style=\"color:#e50d0d\"><strong>Can you see any errors on this? What do you think has led to this gentleman&#8217;s AKI? and What should you do?<\/strong><\/p>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><button id=\"at-98404\" class=\"c-accordion__title js-accordion-controller c-accordion__title--button\" role=\"button\"><strong>Reveal<\/strong><\/button><div id=\"ac-98404\" class=\"c-accordion__content\">\n<p>Both empagliflozin and ramipril have been signed for and given when they should have been omitted. <\/p>\n\n\n\n<p>The AKI is likely to be multifactorial due to sepsis and medication driven with gentamicin, empagliflozin and ramipril all being culprits that lead to AKI in the unwell patient.<\/p>\n\n\n\n<p>So what should you do?<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>Stop Gentamicin, it is unclear if this is causal but we know it can cause nephrotoxicity and this gentleman&#8217;s kidneys are already unhappy<\/li><li>Encourage him to drink\/IV rehydration. This gentleman has been unwell with a fever so his insensible losses have gone up. If he has been septic it is possible he has been in a fluid deficit in admission and it is possible this has not been corrected sufficiently. If the gentleman is able to drink and strict fluid balance can be undertaken then that may be sufficient or he may need a short time on IV rehydration.<\/li><li>Withhold both empagliflozin and ramipril and monitor blood sugars and blood pressure. Blood sugars may be higher without diabetes medication and you may need advice from the diabetic team to manage this as an inpatient. Blood pressure is likely to rise as this gentleman clinically improves at which point you can consider reintroduction of ACE-inhibitor.<\/li><\/ol>\n<\/div><\/div>\n\n\n\n<p>You follow the above advise and slowly this gentleman&#8217;s renal function improves. You stop the empagliflozin on the advice of the inpatient diabetes team and await their review as an outpatient and alternative oral medication is started. Ramipril is restarted on discharge with no ill effects. This gentleman has his U+Es rechecked at his GP 2-3 weeks and they have returned to pre-admission values.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Well done you have completed the case.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>You are on your first day as the FY1 on the General medical ward.
