{"id":5413,"date":"2021-03-10T22:48:29","date_gmt":"2021-03-10T22:48:29","guid":{"rendered":"https:\/\/edren.org\/ren\/?page_id=5413"},"modified":"2024-11-15T16:49:51","modified_gmt":"2024-11-15T16:49:51","slug":"sglt2i-how-to-start","status":"publish","type":"page","link":"https:\/\/edren.org\/ren\/handbook\/prescribing-handbook\/general-prescribing-notes\/sglt2i-how-to-start\/","title":{"rendered":"SGLT2i: how to start"},"content":{"rendered":"<p>This page gives advice on initiating treatment with SGLT2 inhibitors (&#8220;flozins&#8221;) in CKD.\u00a0 It is primarily aimed at renal physicians.<\/p>\n<p>&nbsp;<\/p>\n<h5>In T2DM:<\/h5>\n<p><span style=\"text-decoration: underline;\"><strong>Indications:<\/strong><\/span><\/p>\n<p>See our CKD <a href=\"https:\/\/edren.org\/ren\/handbook\/unithdbk\/ckd\/ckd-summary-overview\/#3_Delay_CKD_progression\">treatment algorithms<\/a>.\u00a0 Consider SGLT2i in\u00a0ALL patients with T2DM and CKD (i.e. if eGFR &lt; 60 or uACR &gt; 3).<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>When discussion with the diabetes team is NOT required:<\/strong><\/span><\/p>\n<p>In patients with diabetes, use is now well-established.\u00a0 These drugs do not need to be prescribed or overseen by a diabetologist; they could be prescribed by a nephrologist, general physician or general practitioner.<\/p>\n<p>Many patients with T2DM will be managed in primary care.\u00a0\u00a0Patients on diet only or diabetes drugs that are unlikely to cause hypoglycaemia in combination with SGTL2i &#8211; e.g. metformin, DPP4 inhibitors, GLP-1, pioglitazone &#8211; don\u2019t need any extra monitoring or medication adjustment.\u00a0 In such patients, an SGLT2i could &#8211; and should &#8211; be initiated without any discussion with the diabetes team.<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>When discussion with the diabetes team would be helpful:<\/strong><\/span><\/p>\n<p>In short: for patients on insulin or a sulphonylurea.<\/p>\n<p>Large improvements in glycaemic control can be anticipated when starting an SGLT2i in patients with good renal function (eGFR &gt; 60).\u00a0 If the patient is already taking insulin or a sulphonylurea then there may be a risk of hypoglycaemia and therefore close blood glucose monitoring will be required; if control was tight prior to starting the SGLT2i then the insulin or sulphonylurea might need to be stopped before starting the SGLT2i.\u00a0 In such patients, blood glucose monitoring and advice on treatment of hypos would be appropriate; discussion with diabetes doctors or nurses may help, especially if the patient is already under secondary care.\u00a0 There may also be a risk of ketoacidosis in patients with longstanding T2DM \/ \u03b2-cell failure.<\/p>\n<p>&nbsp;<\/p>\n<h5>In T1DM:<\/h5>\n<p>SGLT2i are used very infrequently in T1DM and only ever under direct specialist supervision by a diabetologist.<\/p>\n<p>&nbsp;<\/p>\n<h5>In non-diabetic chronic kidney disease:<\/h5>\n<p><span style=\"text-decoration: underline;\"><strong>Indications and contraindications:<\/strong><\/span><\/p>\n<p>See our CKD <a href=\"https:\/\/edren.org\/ren\/handbook\/unithdbk\/ckd\/ckd-summary-overview\/#3_Delay_CKD_progression\">treatment algorithms<\/a>.\u00a0 Consider SGLT2i in patients without diabetes if CKD and uACR &gt; 25 mg\/mmol (uPCR &gt; 50).\u00a0 Such patients would usually also be prescribed maximum-dose RASi (unless contraindicated).<\/p>\n<p>SGLT2i effect on blood glucose diminishes as eGFR falls but efficacy at preventing CKD progression and cardiovascular outcomes does not.\u00a0 Their efficacy at glucose-lowering falls off significantly at GFR &lt; 45 ml\/min.\u00a0 In patients already taking SGLT2i, these agents can be continued as eGFR falls below this threshold but should be discontinued at the start of dialysis therapy or time of kidney transplant.\u00a0 SGLT2i can be initiated for CKD at any eGFR &gt; 15 ml\/min.<\/p>\n<p>Relative contraindications include immunosuppression (e.g. kidney transplant, lupus, vasculitis) and ADPKD.<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>How to prescribe:<\/strong><\/span><\/p>\n<p>The MHRA have <a href=\"https:\/\/www.medicines.org.uk\/EMC\/medicine\/27188#INDICATIONS\">licensed dapagliflozin<\/a> (10 mg once daily) for use in CKD.\u00a0 The license permits initiation in patients with eGFR 15 ml\/min or above.\u00a0\u00a0SGLT2i are now on the NHS Lothian formulary for T2DM, HFrEF and CKD.\u00a0 (The East Region Formulary Committee approved Dapagliflozin for use in CKD on 25 May 2022.)<\/p>\n<p>Empagliflozin has also been licensed for use in CKD and approved for this indication by the SMC.\u00a0 (An application for this to be added to the East Region Formulary will be submitted in November 2024.)<\/p>\n<p>RIE pharmacy stocks empagliflozin, canagliflozin and dapagliflozin.\u00a0 It is highly likely that the renal and cardiovascular benefits (and side-effect profile) is similar for all SGLT2i &#8211; i.e. it is a <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/clc.23508\">class effect<\/a>.<\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"text-decoration: underline;\"><strong>Do I need to monitor anything?<\/strong><\/span><\/p>\n<p>There is no need to routinely monitor blood glucose or eGFR after initiation.\u00a0 Anticipate a haemodynamic drop in eGFR &#8211;
